Cause. In IMPA, the immune system is inappropriately activated to send white blood cells to the joints. The white blood cells release chemicals and enzymes into the fluid that bathes the joints, disrupting the protective function of this fluid.
Glucocorticoids are the cornerstone of treatment for Type I Canine IMPA. It has been shown that treatment with prednisone results in remission of 50% of cases with Type I IMPA. Immunosuppressive doses are used for the initial 2 weeks.
For example, in studies of dogs presented for further investigation related to fever of unknown origin, 8% to 40% were diagnosed with IMPA and many had no apparent joint pain or swelling.
Conclusions and clinical relevance: Although the number of dogs evaluated was small, limiting generalizability, results of this study suggested that cyclosporine offers promise as a suitable alternative to prednisone for treatment of IMPA in dogs.
